These type of changes need to go through the Institution Database Advisory Team (IDAT), please complete an Institution Request form which can be found at: http://www.ode.state.or.us/pubs/instID/institutionrequestform1380a.docx then email or fax it to us at ode.institutions-request@ode.state.or.us or 7503.378.5156.
